Memory Care Consultations

Meet one-to-one with a geriatric social worker to discuss memory care needs. Receive information and referrals personalized to your family’s situation, wherever you are in your dementia care journey. Our caring staff can assess the needs and abilities of the person with dementia to provide targeted guidance and/or meet separately with the family caregiver to provide information and support.

Copies of The 36 Hour Day: A Family Guide to Caring for People Who Have Alzheimer’s Disease, Other Dementias and Memory Loss by Nancy Mace and Peter Rabins will be provided courtesy of Right at Home Nassau Suffolk.

SPARK! A Program For People Experiencing Early Memory Loss

Designed for those experiencing early memory loss, this program will provide education, stimulation, and interaction with others with mild cognitive impairments, in order to maintain confidence, independence, and dignity for as long as possible.

The Felice Buckvar Reflections Program Presents: Memory Connection

A socialization & mental stimulation program for people with early to moderate dementia.

A typical day includes: Welcome Activity, such as trivia or word games, while enjoying coffee, tea and light refreshment.
Stimulation Stations with fun puzzles and games in ability groups that build on each individual’s strengths.
Lunch provided from our Café.
Featured Program, such as musical entertainment, art programs, or guest speakers.

An individual Memory Care Consultation is required before attending for the first time to ensure proper matching of person and group.
